Will the ASQ-3 results for each of the domains be in the same order for every language (i.e., communication, gross motor, fine motor, problem solving, social)? We have a template with the domains in our electronic health record, but many of our staff won't actually be able to read the forms in other languages.
Yes, the domains on the Information Summary sheets for every translation follow the same order as the English. The translations all mirror the English so that someone who doesn’t speak the language can follow along if they choose.
Where do I indicate that the child is premature on the ASQ-3 Summary report?
To indicate that a child was born prematurely, you would simply bubble in “Yes” for the statement “Was age adjusted for prematurity when selecting questionnaire?” This is located in the top portion of the ASQ-3 Information Summary sheet.
