What is the difference between the long and short form for entering a new screening?

The long form is meant for a screening that is being completed at the time that the information is being entered into ASQ Online. For example, if you are completing an ASQ-3 screening with a family over the phone, you can enter the results directly into ASQ Online using the long form and the system will automatically score the questionnaire for you and save it to the child's profile. The long form includes all text, pictures, and instructions that are included on the paper questionnaires. 

The short form is meant for entering the results of a screening that has been previously completed on paper. For example, if a family has turned in a completed paper questionnaire and you want to enter the results into your ASQ Online account to store the data, you can use the short form to quickly enter those responses. The short form only includes the item numbers and the responses for each item. The short form does not include the text of the questions, any illustrations, or additional instructions. Once the responses have been entered, the system will automatically score the questionnaire and save it to the child's profile.
