What if my ASQ keycode is missing or lost?

If you lose your ASQ keycode, contact Brookes Publishing via the Customer Support Center or by phone (1-800-638-3775). If you are an international customer, contact information can be found here. We will email you a Keycode Replacement Request Form. You can also download the Keycode Replacement Request Form here.

Copy the ASQ Keycode Replacement Request Form onto your organization’s letterhead. Complete the form and get the signature of your Program Administrator. Fax or mail the completed form and a valid proof of purchase (an original receipt or a photocopy of the copyright page) back to Brookes, to the attention of Customer Service (ASQ Keycode Replacement Assistance)

A replacement keycode will then be sent to the email address you provided when you filled out the form.
