You may add your program’s logo and contact information to the Family Information Sheet and/or remove the drawing of the mother and infant without seeking permission from Brookes Publishing. This is detailed in the ASQ-3 User’s Guide, beginning at the bottom of page 48, “At the top right of each family information sheet is an illustration of a mother and infant. Programs may use this space to place their program logo or contact information if desired….the family information sheet provides space to insert, stamp, type, or write essential identifying information for the program….The bottom of each version of the family information sheet has a section for information important to the program….”
Any other modification or reformatting of the Family Information Sheet needs the written approval of Brookes Publishing. Making changes, additions, or deletions to, or reformatting the other questionnaire pages (any or all of the pages following the Family Information Sheet) requires the written approval of Brookes Publishing.
