Currently, our child care teachers fill out an ASQ-3 on children prior to parent-teacher conferences while a home visitor has parents fill out an ASQ-3 at home in the same timeframe. The 2 separate questionnaires can be discussed at the parent-teacher conference. However, we screen twice a year and having both teachers and parents complete the questionnaires means that there is a possibility of a teacher completing the same ASQ-3 interval two times and the parent completing it twice as well.
It is great to have both parents and child care providers complete the questionnaires and discuss results as your program is doing. As long as a location owns an ASQ CD-ROM, the PDF files are able to be saved on as many computers as you'd like at that physical location. There is no limit. The files can also be uploaded to a server accessible to people at that site (but not a server that is accessible by people across multiple physical locations).
I would prefer ASQ-3 Learning Activities that can be used with both boys and girls. For example, one of the activities in Gross Motor 0-2 months says, "...while your baby is on his tummy, lie beside him..." What if I am giving this activity to the parents of a girl? Do you have an option that uses "him or her"?

Don't worry! Inside each ASQ-3 or ASQ:SE-2 box, under the flap, there is a CD-ROM with PDF files of each questionnaire interval. You can print copies of the questionnaire straight from the CD-ROM.
